the institute of medicine has estimated that medical errors are responsible for up to 98,000 deaths per year. The joint commission expects healthcare organization to identify and respond appropriately to significant medical mistakes considered to be "sentinel events." root cause analysis is a process that identifies the basic underlying factors and or causes of an error. This course will provide participants with an understanding of the root cause analysis process and how it can be utilized to prevent future errors. Participants will be guided through the steps of the root cause analysis from the initial step of "forming a root cause analysis team" and conclude with the creation of an "action plan" which will institute changes to reduce future errors.

Upon completion of this course participants will be able to:

Be familiar with the joint commission standards concerning sentinel events and root cause analysis.
Understand the differences between "proximate or direct causes" and "systems or root causes" of events.
Know the steps involved in the root cause analysis process

The course was 45 minutes and $30 but it is gone and yes you did get a certificate. The link now takes you to a 120 hour program. It is something like the graduate certificate I completed from UF.
