Does anyone know how to properly reference the following dissertation on a CV: The reason I'm asking is because I had ProQuest publish it in book form for me as a final copy. Any thoughts are welcomed.

If you go to any top-notch university website you'll find that the faculty all have their CVs online. There may be some small differences but I'm guessing that you'll quickly discover the standard method.
