This site gives a listing of all CW PhD programs. You may have to dig through a bit. Also, while not purely a CW program, both Texas Tech and Old Dominion have PhD programs in either Communication/Rhetoric or English available online.

Yes. Keep in mind that an MFA in CW is normally considered a terminal degree, which is why you don't see a lot of PhD's available.
The Texas Tech program is sorta totally DL. All course work and the dissertation are done DL. But you do have to attend a conference each May. I think it is a couple of weeks (and the cost this past year was about $1600 for the conference). The conference is at TTU in Lubbock, Texas.
