Needs registration form for gre subject test!

Can anyone help me with a link to download registration form for gre subject test, to European applicant?
I went to gre download site and couldn’t see a form for subject test.
Also, want to know, if they have a PC test for psychology?

I was there, but unfortunately I don’t see the right form for my choice.
The “1004-2005 INTERNATIONAL TEST SCHEDULING FORM is for Computer-Based General Test.

I would like to download a test scheduling form for subject test. I need that as soon as possible. May be I don’t go through because I skip this credit card payment. I shall pay by using other means.
