Which DL MBA indexes use last 60 GPA?

Do you know which schools have a DL MBA program where the admissions index uses the last 60 credits GPA (as opposed to the cumulative GPA)?

For example:
the University of Texas System uses the following index --
(GMAT + (last 60 GPA * 200)) > 1080
