Question for non-US degree holders applying in the US

A question for holders of non-US degrees.

When applying for a position with a US company, how is the credential translation typically handled? I can picture 3 cases.
*The applicant has an AACRAO or equivalent evaluation in hand and submits with the application.
*The hiring organization requests that the applicant provide evidence of equivalency with the required degree.
*The hiring organization does the review internally or hires out the review at their expense.
